Innocent Vareed Thekkethala: A Tribute To Actor, Former MP And A Cancer Survivor Who Advocated For Fellow Patients

Home The name “Innocent” is popular amongst Malayali households. Four generations grew up watching and enjoying excellent comedic performances and memorable characters portrayed by the late actor. During Malayalam cinema’s golden heyday in the 1980s and 1990s, practically every film included a few recognised faces, and Innocent was one of them. His passing dims the light of a generation of performers who produced a wave of outstanding performances. Innocent Vareed Thekkethala has a 50-year career in Malayalam film. He also appeared in films in Tamil, Kannada, Hindi, and English. Innocent was a best-selling author as well as a Member of Parliament (MP) in India’s 16th Lok Sabha (2014-2019), representing Kerala’s Chalakudy seat. The actor was also a cancer survivor who, unlike many of his contemporaries who sought treatment in silence, was open about his illness and raised awareness.  “>”>His cancer diagnosis Innocent had mentioned how he felt an unusual throat pain about a decade ago. He got himself tested and underwent a biopsy. He was in a film shoot when noted oncologist Dr VP Gangadharan informed him of a “news.” Innocent could sense that it was something bad but found the inner strength to memorise the lines he had in that scene and completed the shoot. Afterwards, he was told that he had non-Hodgkin’s lymphoma, a type of blood cancer. He had noted how such information causes a flurry of racing thoughts in one’s mind, including the realisation that he must leave his loved ones behind. He did, however, find the inner strength to overcome the condition. “>”>What is non-Hodgkin’s lymphoma?  Non-Hodgkin’s lymphoma is a blood cancer that begins in the human body’s lymphatic system, where the lymphocytes, a type of white blood cell (WBC), start to divide and grow abnormally into the bloodstream. These lymphocytes can form tumours throughout the body. According to research, 1 out of every 43 men and 1 out of every 53 women can develop non-Hodgkin’s lymphoma in their lifetime. However, the epidemiology of Non-Hodgkin’s Lymphoma in India is less when compared to Europe and North America. In India, the age-adjusted incidence rates for Non-Hodgkin’s lymphoma in men and women are 2.9/100,000 and 1.5/100,000, respectively. “>”>What are the symptoms of non-Hodgkin’s lymphoma?  The most visible symptom of non-Hodgkin’s lymphoma is a swollen lymph node in the neck, armpit or groin region. Oral lesions of non-Hodgkin’s lymphoma manifest in the form of tooth mobility, swelling or ulcer, unexplained dental pain, or lesions. The late actor underwent chemotherapy to overcome the disease. While he was being treated, his wife Alice underwent screening and testing for cancer, including mammography. The mammography revealed that she had breast cancer. Breast cancer is the most commonly reported cancer in women, particularly older ones. Breast cancer is relatively easy to cure compared to other cancers because it can be surgically removed without causing major implications in the human body aside from physical appearance. The most common type of breast cancer is ductal carcinoma in situ. Women are advised to begin treatment as soon as possible to avoid it becoming invasive. Lobular carcinoma in situ (LCIS) : This is a condition in which cells within the breast’s milk-producing glands or lobules are abnormal. LCIS is not cancer, but it increases the cancer risk, so women should have regular checkups and treatment. Different types of invasive Breast cancer types are as follows; Invasive ductal carcinoma (IDC) : It is the most common form of breast cancer, reported in at least 80% of all breast cancer, according to statistics published in cancer.org. It is a malignant tumour that begins in the breast milk ducts and can spread to other parts of the body through the lymph nodes. Its symptoms include a lump in the breast, nipple discharge, and changes in the appearance of the breasts. Invasive lobular carcinoma : It is a condition where the breast’s lobules are affected. It is usually detected on imaging tests such as mammography, ultrasound, and MRI. According to the study published by cancer.org, one out of every ten reported breast cancer is an ILC. Rare types of breast cancer Inflammatory breast cancer (IBC): It is an aggressive form of invasive breast cancer that causes symptoms such as redness and swelling as a result of cancer cells blocking the skin’s lymphatic vessels. Paget s disease of the nipple: It is a rare type of cancer that affects the nipple or the areola. Common symptoms include hard, lumpy or crusty nipples or the area around the nipples. It is also characterized by the redness, discharge or bleeding from the nipple and the appearance of a lump. Phyllodes tumour: It is a rare form of breast cancer where the tumour is formed in the connective tissue. Phyllodes tumours can be both benign or malignant. Metaplastic breast cancer (MBC): It is a rare type of breast cancer that can be more aggressive than others. It is characterised by the variety of cell types that make up the tumour, which makes MBC difficult to diagnose. What are the different stages of breast cancer? Breast cancer stages are classified based on the severity of the cancer and the extent to which it has spread to other body parts. The treatment course is determined by the cancer stage and the location of the tumour. Estrogen and progesterone play a pivotal role in the reproduction and sexual function of women. Estrogen consists of estrone, estradiol, and estriol. Like any other hormone, an excess of estrogen is a double-edged sword that puts the body at risk of several health issues, including cancer. As per cancers that develop due to estrogen, known as estrogen-dependant, include breast, ovarian and uterine cancer. CancerMitr spoke to Dr Sandip Bipte regarding estrogen dependant cancer, its prevalence and how to handle it. Dr Sandip Bipte specialises in oncoplastic breast surgery and has over 17 years of experience as a specialist. He shared his expertise with the CancerMitr team about the prevalence of cancer, notably breast cancer, due to the hormone estrogen. What role does estrogen play in the development of cancer? Estrogen is a hormone secreted by ovaries, adrenal glands and special fat tissues. Cancer is basically the abnormal lumps of tissue known as neoplasm or tumour that is malignant in nature – which means it grows, spreads and invades other body parts. Like normal body cells, cancer cells also have special proteins called receptors. If the cancer cells are estrogen receptor-positive (ER-positive), they bind to estrogen. Estrogen fuels the growth and spread of cancer cells. Estrogen-dependant cancer includes breast cancer, ovarian cancer, and uterine cancer. 60% of breast cancer cells are ER-positive, in my experience. Does estrogen trigger the growth of the cancers mentioned above? Yes. Excess estrogen levels trigger the cells, especially breast cancer cells. It can start with a small tumour or mutation; if it is ER-positive, the tumour size increases over time. So, is it safe to call estrogen a carcinogen? ​ No. Of course not. Although it can trigger cancer cell proliferation, it is not right to call estrogen carcinogenic. Estrogen is essential for various functions in the human body, associated with sexual and reproductive health. It improves the conditions of the ovaries, vagina, and secondary sexual characteristics. It is the excess estrogen levels that cause health concerns, and cancer is one of them. Excess estrogen levels cause other health-related challenges like mood swings, blood clots, irregular periods, etc. In fact, all hormones are harmful in excess. For example, Androgen (male hormone) is linked with prostate cancer. A perfect hormonal balance is crucial. This is because the human body relies on these chemical messengers, and if excessive hormone secretion is causing physical or mental health problems, it must be treated. How do you find out if a particular cancer is estrogen dependent? The cancer cells are taken out during the biopsy and checked under laboratory conditions. If it has estrogen receptors, then it is called ER-positive. If it has progesterone receptors, then it is called PR-positive. If it does not have either of these receptors, then it is called ER-PR negative. Women with ER-positive cancers are given treatment for estrogen imbalance, like surgical removal of ovaries (which secretes estrogen) or medication. If excess estrogen does cause cancer in both women and men, wouldn’t it be a concern for those to undergo HRT? It is important to note the incidence of breast cancer in men is low compared to women. Women are vulnerable to breast cancer, ovarian cancer, and uterine cancer. Yes, HRT or hormone replacement therapy is a concern. HRT is a treatment involving estrogen and progesterone to treat health issues due to their deficiency. However, a medical expert always ensures that the HRT process is done systematically and healthily. Even then, there are cases where women undergo HRT without proper medical supervision and risk health issues due to excess estrogen and progesterone, including cancer. It is because PCOD as a condition is associated with estrogen dominance or high levels of estrogen. Human body consists of different types of cells. Mitosis is the process by which these cells divide to form new cells, duplicating their contents. In a typical process, aged cells die and are replaced by fresh ones. Tumours are caused by abnormal mitosis, where the cells do not die but continue to divide.  Tumour or Neoplasm is a mass or collection of aberrant cells that can develop in any area of the human body.  Tumours can be either benign or malignant. What is a benign tumour? Benign or non-cancerous tumours remain localised in their region and do not spread or invade other human body parts.  Benign tumours need not be life-threatening, and a person can survive without treatment. However, its abnormal growth can press other organs, causing damage. Furthermore, certain benign tumours can be precancerous, which means they can progress to cancer if not treated. What are the different types of benign tumours? Adenoma: It forms in the epithelial tissue, which is a layer of thin tissue layer covering glands, organs, and others. Adenomas tend to be precancerous in nature and should be treated.  Lipoma: They are soft tumours that develop in fat tissue and are most commonly found in the back, shoulders, arms, or neck. They do not have to be precancerous, but they can be distressing for the patient and necessitate treatment. Myoma: It develops from the walls of blood vessels or muscles. Uterine fibroid is the name given to a myoma that forms in the uterus. Fibroids: They develop in any organ, ligament, or tendon’s connective or fibrous tissue. Although they are rarely malignant, they must be treated if they cause severe discomfort or other signs like vaginal bleeding. Uterine fibroid, uterine myoma, or leiomyoma are all terms used to describe uterine fibroids. Nevi: They are benign skin growths that can eventually turn into skin cancer, especially the dysplastic nevi. They can develop into cancerous brain tumours over time.  Neuromas: It grows in the nerve cells at any part of the human body. Osteomas: They are the benign growth of new or existing bones, also referred to as exostosis. An osteochondroma is a bone growth that is covered with cartilage. They need not become cancerous, but osteomas tend to be very painful and require treatment. What is a malignant tumour? Malignant or cancerous tumours are dangerous and life-threatening. They divided aggressively and spread from one part of the human body to another, invading and causing damage, a condition called metastasis. What are the different types of malignant tumours?
